Case Study: Patient Support App for people living with type 2 diabetes on a self-injection medication

Background

The Challenge

  • Large proportions of people living with Type 2 Diabetes are not meeting treatment goals set by their Healthcare Professional
  • Only 17% of youth and 21% of adults with diabetes are meeting target HbA1c levels
  • New treatments that target hormones involved in blood-sugar control are effective, but persistence can be low, especially in the weeks following initiation, due to GI-related side-effects

Our Behavior Change Approach

  • We utilized Health Psychology and Behavior Change Techniques to develop personalized on-demand content for patients to:
    • Empower understanding of new treatment to increase likelihood of adherence and help navigate potential symptoms or challenges in the early weeks after treatment initiation
    • Support achievement of goals based on their personal values to help establish long-term medication taking routine and sustainable self-management behaviors
  • We provided support for early clinical experience of treatment for healthcare professionals to reduce workload burden and training for patients to self-inject their medication confidently

The Solution

Using Atlantis Health's proprietary personalization algorithm and our configurable Patient Support App, patients are supported with:

  • Digital coaching tailored to the individual's needs
  • Behavior change modules with interactive lifestyle and treatment content
  • Targeted reminders for dosing, blood glucose monitoring,  next prescription
  • Escalation pathways for optional nurse support

The Impact

  • More than 400 patient app downloads in Australia within 6 weeks of launch
  • >75% of patients accessed and engaged with the learning modules immediately
  • 73% of patients set up in-app reminders (medication reminders and trackers)
  • Less than 1% of patients required additional nurse support through escalation pathways
