The 'Life After Heart Attack' patient support programme is designed for patients diagnosed with Acute Coronary Syndrome (ACS) and prescribed any type of oral anti-platelet medication. This six-month intervention is personalised based upon assessing and addressing each individual patient's beliefs which have been shown to be associated with non-adherence.
